Breast Rippling: If Symptoms Appear, Do You Need Revision Surgery? (Advice from 20 Years of Research)
Hello — we are U&U Plastic Surgery, a clinic devoted exclusively to breast surgery.

About 10 years ago, our breast surgeons appeared on the TV program 'Let Me In'

This article was not written simply to promote our clinic.
It was written to provide useful information both to those preparing for a first surgery and to those who have had surgery and now suspect rippling.
As the title suggests, let us first address whether revision surgery is necessary when rippling occurs.
If rippling symptoms
appear, is revision surgery a must?
Source
The answer: it does not always have to go as far as revision surgery.
In some cases, improvement is possible with fat grafting alone.
Then if rippling appears,
when is revision surgery needed,
and when can fat grafting be enough?

We will cover this in detail below.
Using published research, we have organized the treatment options by symptom severity — please be sure to read on.
Rippling symptoms:
treatment by severity
Source

The material above comes from a respected journal in the international plastic surgery community.
The study was written to organize the causes of rippling and the treatments appropriate to each level of severity.
It is referenced in actual clinical practice, so you can consider it a trustworthy source.
The paper classifies rippling symptoms into three grades.
1) Grade 1 (mild)
: palpable but not visible
2) Grade 2 (moderate)
: not visible normally, but implant folds show when bending forward
3) Grade 3 (severe)
: implant folds and contours are noticeable even when standing.
For each grade, the recommended management is summarized as follows.
Grade
Direction of treatment
Grade 1
(mild)
Non-surgical management first
- small-volume fat grafting if needed
Grade 2
(moderate)
Partial corrective treatment recommended
- fat grafting
- acellular dermal matrix
- fascial flap if needed
Grade 3
(severe)
Surgical correction
- implant replacement
- implant repositioning

Advice for those worried they can
never diet again
Rippling symptoms
Even if the rippling is corrected,
won't it just come back
if I lose weight later?
I'm worried I'll have to live without
ever dieting the way I want..
Source
If you lose weight, rippling is indeed possible.
But the problem mainly arises when body fat is lost rapidly.
Rippling occurs when the subcutaneous fat layer directly over the implant is thin. In other words, the danger zone is 'losing so much weight that even the skin over the breast thins out.'

But dieting seems to
shrink the breasts too —
how do you lose weight without
affecting the subcutaneous fat layer..?
Source
Admittedly, dieting without affecting the breasts at all is difficult.
But there is a way to diet while protecting the breast fat layer as much as possible:
a slow, gradual diet.
When weight drops rapidly, the body switches into 'emergency mode.' Put simply, it is set to burn the most readily available fat first.
The most readily available fat is subcutaneous fat. That is why crash diets often shrink the breasts first — the body draws on the breast's subcutaneous fat.
But with a slow diet, the order in which fat is burned changes. Instead of subcutaneous fat first, fat comes off evenly — from the abdomen, thighs, and elsewhere.
So if your breast skin is thin, we strongly recommend avoiding crash diets.
