First, the removal procedure itself can be more complex.
Augmentation places a new implant / removal takes out one that is already in place.
In other words, augmentation is performed when the breast tissue is more elastic and blood flow is better.
Removal, on the other hand, is performed under more demanding conditions.
The older the implant, the more likely the capsule around it has developed inflammation or calcification.
Because the implant must be removed while separating this capsule, the procedure can be technically more demanding.

When it comes to risk, however, removal can actually be lower.
The complications most often mentioned in breast augmentation involve the capsule and infection.
Since removal takes out the implant that would cause capsule problems and infection, the risk can actually be lower.
