The reason breasts sag with pregnancy and childbirth is simple.
As the breasts enlarge during this period, the supporting ligaments that hold them in place become physically stretched.
However, once stretched, these supporting ligaments do not return to their original state after childbirth.
This fact is also clearly stated in the chapter on breast aging in a globally recognized plastic surgery textbook (Oxford).

To date, no non-surgical method has emerged that can restore stretched breast-supporting tissue to its original state.
